"Why is str_shuffle() used so frequently? " - Because a lot of programmers, especially PHP programmers, don't know about security. A programmer wants a random token and that function seems to do that just fine. Even if they're told "it's not random enough", as long as they can't find a pattern in it, they will dismiss it as good enough.
