Wordlist generator with a certain pattern
You could use a tool like Generex to generate strings out of a regular expression. I think your case would be:
[Aa]meri\d{3}\s?[Cc]a\s{0,4}#

Limiting five letter string to only include words from dictionary in Crunch?
It was easier to create two different lists and combined them, i think. In Python:

Crunch password list, no duplicate characters
Crunch is not designed for that option. It also doesn't make sense that it would. Password list generators need to create very large lists that match patterns. Your request is a constraint.

Piping crunch into hydra
I think, after a few searches, that it's not possible as hydra doesn't read stdin by design, and there are no options to use in order to achieve this.
But it can be a great improvement indeed.

Generating wordlists with upper and lower case with crunch
I'm unsure if crunch is capable of that. Why don't you want to use a scripting language?
Here is a Python solution:
from itertools import product, permutations
words = ['478', '1990', 'gmgm', '...
